All Phoenix Children's Chorus staff, board members, and other adult participants are required to sign this liability waiver before participating in Phoenix Children's Chorus activities, events, rehearsals, camps, tours, or other programs. Participation is contingent upon receipt of a completed and signed waiver.

I will be participating in Phoenix Children's Chorus activities, including but not limited to rehearsals, performances, events, choir camp, and domestic and international tours. In consideration of being permitted to participate in these activities, I acknowledge and fully understand the risks and hazards that may be associated with participation, including domestic and international travel. On behalf of myself, my heirs, executors, administrators, and personal representatives, I hereby release, waive, and hold harmless Phoenix Children's Chorus, Phoenix Bible Church, Lost Canyon Camp, the City of Phoenix, Highland Lakes School, Hidden Hills Elementary, Accent Concerts, and their respective officers, directors, employees, agents, and volunteers from and against any and all claims, demands, causes of action, damages, losses, or liabilities for personal injury, illness, death, or damage to personal property arising out of or related to my participation, except to the extent caused by the negligence or willful misconduct of the released parties.

I, the undersigned, further acknowledge and agree to reimburse and hold harmless the organizations listed above from any financial liability, claims, or obligations arising from injury, damage, or loss to the person or property of others caused by my actions. I further acknowledge and understand that Phoenix Children's Chorus reserves the right to terminate my participation in any Phoenix Children's Chorus activity, event, camp, or tour if I fail to comply with the organization's rules, standards, policies, or instructions. In such an event, I understand that I may be required to leave the activity at my own expense and that no fees, expenses, or costs paid to Phoenix Children's Chorus will be refunded.

I HAVE READ THIS WAIVER OF ALL CLAIMS, RELEASE FROM LIABILITY AND ASSUMPTION OF RISK. I FULLY ACKNOWLEDGE AND UNDERSTAND THE SAME, AND AGREE TO ITS TERMS.
