General Permission Slip - Staff/Board

Participation Permission *

I am participating voluntarily as a staff member or board member with Phoenix Children's Chorus. I have the authority to enter into this agreement on my own behalf. I consent to participate in all Phoenix Children's Chorus programs and activities, including rehearsals, performances, events, camp, tours, meetings, and other activities conducted and/or sponsored by Phoenix Children's Chorus. I also grant Phoenix Children's Chorus permission to photograph, record, and publish photos, videos, and other media of me for marketing, promotional, educational, and advertising purposes.

I need the following special accommodations:

In the event of illness or injury, I authorize Phoenix Children's Chorus to obtain or arrange for emergency medical care on my behalf if necessary. I consent to the administration of medication and treatment as prescribed by my physician or as determined by an available physician, nurse, other licensed healthcare professional, or qualified first aid provider.

Please note: For touring choirs, we take medical personnel with us for the safety of the choristers, chaperones, and staff. If you select no, please be aware that a conversation must be had with the staff and medical team in order to understand your wishes while also ensuring that we can make safe and emergent decisions while on tour.
