Cadenza/Encore Tour Deviation Form

This is a mandatory form for choristers, chaperones, and staff members to indicate if you will or will not be returning with Phoenix Children's Chorus at the end of our tour. Finalized dates will be announced at the meeting this Saturday, so this form can be filled out once those are announced. Decisions are final after 2/27/26.

If you indicated that you will not be returning with PCC and then decide you want to return with the group after February 27th, you will be responsible for arranging your return to Phoenix.

If you select that you are returning with the group and decide you will not be retuning with the group after February 27th, you acknowledge that you are forfeiting your right to a flight credit.
