I the undersigned will be a volunteer with Phoenix Children’s Chorus during the 2024-2025 choir season including but not limited to Williams, AZ summer camp August 2024, weekly rehearsals, performances and/or tour. In consideration of being permitted to participate in said choir events, I the undersigned in full recognition and appreciation of the dangers and hazards inherent in domestic travel to which I may be exposed during my participation in said event, do hereby agree for myself, my heirs, and personal representative(s) hereby release and hold harmless Phoenix Children’s Chorus, Lost Canyon - A Young Life Camp, Phoenix Bible Church, Highland Lakes School, and the City of Phoenix and all its officers, agents, employees, and volunteers from and against any and all claims, demands and actions, or causes of action, on account of damage to personal property or personal injury or death which may result from my participation, and which result from causes beyond the control of, and without the fault of negligence of the pervious stated agencies, its officers, agents, employees, or volunteers during the period of my participation as aforesaid.

I, the undersigned further acknowledge and understand that reservations, schedules, itineraries, rates, charges, and other accommodations suggested by the Phoenix Children’s Chorus are based on information provided to the Phoenix Children’s Chorus, and the above mentioned organizations are not responsible or liable to the undersigned for the accuracy or reliability of such information or activities connected therewith such as travel, lodging, meals, government regulations and charges or rates related to such activities or items. I, the undersigned further acknowledge and understand that as a volunteer I may encounter various risks, both known and unknown, from events and activities of such participation and travel, specifically including but not limited to commercial transportation, lodging, acts of God, strikes, insurrection or war, and the undersigned specifically accept and agrees that the above mentioned organizations and its officers, agents, employees, volunteers, insurers, and representatives from any liability for any injury, damage or harm resulting from any risk, event or activity. I, the undersigned further acknowledge and understand to reimburse and hold harmless the above mentioned organizations for any financial liability or obligations for injury and damage to the person or property of others caused by my actions and I, the undersigned further acknowledge and understand that the Phoenix Children’s Chorus has full authority to terminate my participation with the choir for any failure to comply with the rules, standards, and instruction of the Phoenix Children’s Chorus with no refund of fees, and that in such event my volunteer opportunity will be terminated.
